What To Expect At Your First Visit
Please call us to schedule an appointment at (360) 629-4097. Be prepared to have your insurance and patient information available to discuss with our receptionist.
At the time of your first appointment, you will be requested to complete a brief health history and a patient registration form. We will mail these forms to you in advance whenever possible, or you may click on the bolded words below to download and print your own registration and medical history to bring with you to your appointment.
If you are transferring to us from another office, please contact them and have your x-rays and records forwarded to our office. This may well minimize the need for additional x-rays and keep your costs to a minimum.
